You can create custom forms for collecting information when people:
- Join your Community
- Join a team
- Book an activity
- Enter a competition or event
- Buy a membership
You can also set rules in your forms to enforce eligibility e.g. Gender must equal "Male" to join a men's team or Age must be over 18 to book this Zumba class etc. The rules can reference any membership products you have in your Community e.g. must have "Adult Membership" to book this training session.
Create and manage forms by going to Manage > Forms in your left panel menu.
From here you can manage your:
- Community Form - displays when people join your Community
- Team Form - displays when people join any of your teams
- Default Activity Form - displays when people book an activity (can be changed per activity)
- General Forms - forms that you create and assign to any of the above categories or assign manually to any of your Activities, Competitions, and Memberships
If you set a Community Form it will combine with all other forms you create - as everyone needs to become a member of your Community to book or enter any Activities or Competitions. Your Community Form should contain the information you want to collect from everyone. For example you may want to collect Age and Gender from everyone in your Community. These two properties would be added to your Community Form.
Then, you may create a form for your activity bookings that contains Emergency Contact Details. When people book the activity they will see a form containing your Community Form properties + your activity booking fields i.e. Age, Gender and Emergency Contact Details. If you have a Community Form set, when you create a new form the fields of the Community Form will already appear in the form you are creating.
When creating a form you can add any of the following items:
- Standard properties: These are in orange on the Properties panel. Simply drag and drop them into your form
- Custom properties: Ones you have already created or you can create new ones by clicking Create a new Property
- Text: For form headings and text information. Click the Other button on the right of the Properties panel
- Dependency groups: For example if someone answers "Yes" to your "Do you have a disability" field, you could then display a "What type of disability do you have? field
Once you have added your text and properties to your form you can choose to create rules. To create rules on a property click Add Rules next to the property.
The rules screen contains:
- Property - this is the property that the rule is being set against
- Rule - this is how the rule is going to operate and will change depending on the property type, they could be any of the following:
- Equals
- Contains
- Starts/Ends with
- Before/After
- Less/Greater than
- Any one of
- Value - this is the entry the participant must make to be eligible
- for a session to be all Females - Equals (rule) Female(value)
- for an email address - Contains (rule) @ (value)
People must pass these rules to submit the form (and proceed into join, book, enter).
To set Membership Rules click the Membership Rules button at the top of your form. Any membership products you have in your Community will display here. Add as many memberships as you like. Now people must have a valid subscription to any of the memberships you selected to pass through the form. This is an effective way to ensure that all your participants have a valid membership before they book and enter Activities.
Now that you have created forms you'll be able to assign forms to Activities, Competitions, and Memberships as you create / edit them OR you can assign to one of the default categories (Community, team, activity).
