Standard booking forms have been shared with your communities and here is all the data they include:

Standard Playwaze data when a parent or carer creates an account is:

Additional data in the booking form for booking onto holiday Activities may include:

• Address fields
• Emergency Contact Details
• School
• Date of Birth 
• Disability
• Ethnicity
• Primary Spoken Language
• Dietary requirements
• Medical conditions/ allergies
• Name of person for Drop off/pick up 
• Parent Password
• Voucher codes details 
• A Privacy Policy agreement

Please note that registers will contain all data collected when booking. If you're an administrator all this information will be available in your Members and Reporting sections.
